Director of Day Habilitation Services
Overview
Job Title: Director of Day Habilitation Services
Reports To: Executive Director
FLSA Status: FT/Exempt – Salaried
Maximum Weight Lifting Limit: 30 lbs.
Position Summary
Person First New Jersey, LLC anticipates the need for an experienced and dynamic individual to lead the newly created position of Director of Day Habilitation Services to begin on or about June 1, 2019. The Director will be responsible for administering day habilitation services; plans, organizes and coordinates overall operation of the day habilitation facilities. The qualified candidate must have prior experience working with the disabled or vulnerable population, and prior management experience overseeing a multitude of day habilitation facilities. The ideal candidate will possess thorough knowledge of State and Federal Regulations, which govern day habilitation services. Strong leadership skills are a must, including the ability to work with a multitude of staff and management of different populations.
The Organization is interested in candidates with a demonstrated ability interacting and working with individuals/families who are emotionally/behaviorally challenged, and a strong knowledge of DDD’s rules and regulations.
Duties and Responsibilities
- Oversees the hiring, orientation, evaluation, training and supervision of all assigned staff
- Supervises staff; develops program policy and procedure; negotiates with funding agencies; participates in relevant committees; coordinates and acts as liaison with other agencies; establishes and maintains on-going evaluation of program effectiveness and clinical intervention as needed
- Forms a highly effective team to accomplish the Agency’s short and long-term goals
- Delegates decision-making and responsibility to the appropriate level; provides the necessary information and direction
- Interfaces with related community agencies, attends meetings as scheduled and/ or needed to include: the Department of Mental Health and Health Insurance Portability and Accountability Act related issues, as well as the Department of Public Health (Substance Abuse Prevention & Control).
- Measures progress against plans and stated goals as it applies to day habilitation program outcome activities
- Develops, implements, reporting methods consistent with the Agency’s procedures to ensure that data and other information gathered is accurate and reported in a timely manner
- Evaluates the results of overall program operations regularly and systematically, and reports results to senior management
- Collaborates with finance department staff to prepare annual budgets and contract agreements
- Monitors contract performance; reviews monthly and annual financial statements making plans of correction as needed, keeping supervisor apprised of progress and challenges
- Administers day habilitation services; plans, organizes and coordinates overall operation of the day habilitation facilities
- Develops philosophy and objectives including annual goals and objectives for the day habilitation facilities which are compatible with the mission and vision of the Agency.
- Establishes and implements standards which ensure the safe and complete care of the service recipients
- Determines a staffing plan for the day habilitation programs to ensure adequate staffing
- Attends and chairs standing committees as assigned
- Ensures that appropriate Policy and Procedures are developed, which govern day habilitation services
- Assists in the development of the annual budget and ensures the monitoring of the program supplies, inventory systems and approval of requisitions
- Assumes on call responsibilities on a rotating basis with other administrators ensuring the smooth operation of the day habilitation program
- Works with senior management in the planning and development of new day habilitation services and programs
- Adheres to agency standard of ethical behavior as outlined in the Corporate Compliance Program, the Code of Business Conduct and associated policies and guidelines, and all applicable laws and regulations
- Performs such other duties as assigned by the Executive Director
- Adheres to the Personnel Policies established by the senior management, and the Policies and Procedures established by the Executive Director
- Proficiency with Microsoft Office Suite and Knowledge of Databases preferred
- Must pass State and Federal background check and Central Registry check at time of hire

Education
A Bachelor’s Degree or higher in a human services related field.
Work Experience
- A minimum of Five years’ work experience with disabled or vulnerable populations
- Minimum of three years’ supervisory experience overseeing multiple locations
- Demonstrated experience in administering programs which require certification and/or licensure
- Possesses a valid driver’s license with a clean and verifiable driving record
- Experience in the oversight of information collection and specialized monitoring and reporting
- A background in support coordination, particularly at the supervisory level is preferred but not required.
Languages
- English and French are the working languages.
- Fluency in English is a must.
- Fluency in French is desirable.
- Knowledge of the Spanish language is desirable.
- Knowledge of another Language is an advantage.

Position Summary:
Person First New Jersey, LLC is currently seeking an experienced and dynamic individual with strong knowledge and varied experiences to provide support coordination for a diverse body of individuals and families with vision, enthusiasm and strength of character. The ideal candidate will need to demonstrate abilities to communicate effectively, lead with charisma, and inspire excellence from individuals and families.
Interested candidates must possess a NJ driver’s license with a clean driving record, have a minimum of one year’s experience providing support coordination services, and possess a minimum of a bachelor’s degree from an accredited college or university.
The Organization is interested in candidates with a demonstrated ability interacting and working with individuals/families who are emotionally/behaviorally challenged, and a strong knowledge of NJ DDD’s funding and support coordination rules and regulations.
Major responsibilities of the position include, but are not limited to:
- Engaging individuals and their families in the development of plans that assure the needs and wishes of the consumers are addressed and life opportunities expanded.
- Assisting in the choice of services and supports, coordinate the provision of services and supports, and monitor the delivery of services and support.

Accountability
- Completes assessments to identify each person’s needs and desires. This will include the use of formal and informal tools to gather information needed to assist the person to identify their needs and wishes.
- Assists the person to develop their Individual Support Plan and personal budget. Updating the ISP as needed and at least annually. This is to be completed with a person centered approach.
- Supports the person to identify the outcomes they need to achieve and assures that the ISP addresses these.
- Identifies the services that will meet the customer’s needs. Completes the referrals. Coordinates the services the person is receiving. Resolves any barriers to the delivery of service. This involves knowing a full array of options and knowing where to learn of others as needs and interest warrant.
- Monitors the services to assure they are being delivered as planned and that the outcomes that were identified are being achieved. Monitors to assure the person is safe and in the best possible health.
- Assists in identifying and sustaining the person’s support network including family, friends, and associates.
- Assists in arranging for and effectively managing generic resources and generic supports.
- Assists in identifying and developing community resources to preserve the person’s well-being in the home and community.
- Attends training and continues to remain knowledgeable about current best practices in services to support people with various disabilities.
- Trains families and consumers on their rights and due process.
- Develops a professional working relationship with the families and consumers on their caseload. This involves getting to know each person we support.
- Maintains a customer service focus. This involves meeting with consumers and their family at their availability.
